Description of Services:
A program created as a proactive approach to address health and wellness for individuals, families and communities by enhancing protective factors that increase knowledge, skills and attitudes for making healthy choices. The program targets youth from a variety of ethnic, economic, educational and behavioral situations, including but not limited to youth that are currently or previously have been detained in juvenile facilities. The goal of this program is to successfully "reconnect" them, and transition them back into their school, community and home.
