Description of Services:
Services provided: case management, respite care (up to 1 year), employment, short term housing, nutrition and wellness, social support and activities, transportation, medical clinic on site, LCDC and counseling services, laundry, and food. Must be documented homeless and have referral from healthcare facility or clinic and recovering from an injury or medial illness as primary diagnosis, HIV/AIDS secondary. Must be able to care for self. Intake M-F, 10am-4pm.
